If you have any questions about vaccinations, physicals, medications at school, allergies, or 504 medical accommodations, please contact Mrs. Iazzetto. Each District 113A health office is staffed by a Registered Nurse. The nurses provide emergency care to students who are injured at school or during school-related activities.
The nurses also collaborate with parents to address the needs of chronically ill children so that the children can remain in school. Health office procedures and requirements are regulated by Illinois School Code and the Illinois Department of Public Health.

Physical, dental and eye examinations and immunizations, prescribed by State Law, are required of students in the categories below. Parents are requested to have their children examined by a licensed practitioner and to return physical, dental and eye examination forms and immunization records to school before the school year begins. All physical exams and immunizations must be filed by October 15 of the school year, or by district policy, the student will be excluded from school.
Examination and immunizations are also required for 1st-grade students who have not attended District 113A's Kindergarten and students new to the District (transfer records will be considered).

Per the CDC: fever 100.4 F, vomiting, diarrhea, rash, possible bacterial conjunctivitis (pink eye) or respiratory virus symptoms that are worsening and not improving.

Per the CDC: Fever-free x 24 hours and is not using fever reducing medication, vomit-free x 24 hours and able to hold down food/liquids, diarrhea-free x 24 hours. Respiratory virus symptoms have improved.
